New patient and follow up visit intake:
Please provide our office with an email address and we can grant you access to your Electronic Health system platform to fill your New Patient Questionnaire form to help expedite your visit.
Access to personal medical records:
A patient can access their encounter notes, electronically, by providing an email to our staff to be granted access. They should then expect an email from our electronic health records company, which will provide them a series of instructions on how to access their encounter notes.
Alternatively, the patient could ask our front desk staff to provide them with a copy of their record.
